Why it matters

Why Private Health Insurance is Essential in Thailand

Thailand has some of the best private hospitals in Asia — Bumrungrad International in Bangkok is regularly ranked among the top hospitals in the region, attracting medical tourists from around the world. But quality comes at a price.

Without insurance, a straightforward appendectomy costs 80,000–150,000 THB. A road accident resulting in a few nights in ICU can exceed 300,000 THB. Cancer treatment or cardiac surgery can run into several million THB.

Thailand's public healthcare system is not accessible to most foreigners on the same terms as locals. Private health insurance gives you direct access to the best facilities, bilingual staff, cashless billing, and no large upfront payments.

How to Choose Your Health Insurance in Thailand

Annual coverage limit

Minimum recommended: 3–5 million THB. Opt for 10 million THB for comprehensive protection, especially for families or those with medical history.

Direct billing network

Check that your preferred hospital is on the direct billing list. Bumrungrad, Bangkok Hospital and Samitivej accept most major international insurers.

Pre-existing conditions

Some insurers cover pre-existing conditions after a moratorium period (1–3 years). Full disclosure at application is essential to avoid rejected claims.

Geographic coverage

"Asia" plans are cheaper than worldwide plans. If you travel to Europe or North America regularly, choose a worldwide plan (excluding or including the US).

Waiting periods

Typically 30 days for general illness, 6–12 months for specific conditions. No waiting period for accidents. Check the fine print for your situation.

Support & assistance

April International and ACS Ami offer English-language support 24/7. In a medical emergency, communicating in your native language is critical.

Frequently Asked Questions

Everything you need to know before choosing your insurance

Why apply for health insurance via your platform vs. direct insurers or brokers?
Going directly to an insurer gives you one quote, at their price, with no comparison. A traditional broker may have 3–5 partnerships and earn commissions on specific plans. Our platform analyses hundreds of plans instantly and suggests the best match for your age, profile, budget and health status — with zero bias. You compare side by side, apply online 24/7, and access free tools to track your coverage, analyse your contract with AI, manage documents and set renewal reminders. No commitment, no fees, instant quotations.
Why do expats need private health insurance in Thailand?
Thailand's public hospitals are accessible but often crowded, with limited English-language services. The private hospitals where most expats receive care — Bumrungrad, Bangkok Hospital, Samitivej — are world-class but expensive. An appendectomy costs 80,000–150,000 THB; a stay in ICU can exceed 50,000 THB per night. Private health insurance gives you immediate access to the best facilities with direct billing, protecting you from financial shock.
What is the difference between IPD and OPD coverage?
IPD (Inpatient Department) covers care requiring at least one night in hospital: surgery, serious illness, ICU. OPD (Outpatient Department) covers consultations without hospitalization: GP visits, specialists, lab tests, medication. Plans with OPD are more expensive but more comprehensive. IPD-only is the minimum recommended; add OPD if you visit doctors regularly.
How much annual coverage do I need in Thailand?
We recommend a minimum of 3–5 million THB annual coverage. Top private hospitals charge international rates, and complex procedures can run into the millions. For families or those with pre-existing conditions, 10 million THB or worldwide coverage gives optimal peace of mind.
Can I get covered with pre-existing conditions?
Yes, but with exclusions or loadings on known conditions. April International and ACS Ami offer tailored plans with medical underwriting. Some insurers cover pre-existing conditions after a moratorium period (2–3 years). Always disclose fully at application to avoid claim rejections later.
Does Thailand require health insurance for my visa?
For the Retirement Visa (Non-Immigrant O-A), health insurance is mandatory since 2019: minimum 40,000 THB outpatient and 400,000 THB inpatient coverage. For other visas (tourist, business, LTR digital nomad), insurance is not legally required but strongly recommended.
Which hospitals accept direct billing in Thailand?
Most major insurers have direct billing (cashless) agreements with Bumrungrad International, Bangkok Hospital (all locations), Samitivej, BNH, Bangkok Phuket Hospital and many others. You present your insurance card and the hospital bills the insurer directly. For out-of-network hospitals, you pay and submit for reimbursement.
How much does expat health insurance cost in Thailand?
Costs depend primarily on age and coverage level. Approximate annual premiums: age 25–35 IPD only: 15,000–25,000 THB. Age 35–50 IPD+OPD: 30,000–60,000 THB. Comprehensive international plan (IPD+OPD+dental+evacuation): 50,000–120,000 THB depending on age. Get your personalised quote for exact pricing.
Does health insurance cover COVID-19 and tropical diseases?
Since 2021, most insurers cover COVID-19 as any other illness within standard health plans. Tropical diseases (dengue, malaria, hepatitis) are generally covered. Always review specific clauses — we help you compare plans on this point.
Can I still get covered if I am over 60 or 65?
Yes — several insurers accept new applicants up to age 70 or even 75. Premiums are higher and some conditions may be excluded. April International and Pacific Cross have senior-friendly expat plans. The earlier you apply, the better your coverage terms.
Am I covered when I travel or return home?
Most expat health plans provide worldwide coverage (excluding the USA for some plans). Whether you're back in the UK, travelling in Southeast Asia, or visiting family in Europe, your coverage follows you. Out-of-network care is reimbursed upon submission of bills and medical records.
